Signs that warrant an evaluation:
Persistent fatigue from anemia that doesn't improve
When anemia doesn't respond to iron or the usual causes, and appears alongside other low values on a blood count, it warrants a bone marrow evaluation.
Frequent infections from low white blood cells
When the count of healthy white blood cells is low (neutropenia), the body becomes more exposed to repeated infections.
Bruising or bleeding from low platelets
A low platelet count can go unnoticed until easy bruising or bleeding that takes a long time to stop appears.
An incidental finding on a routine exam
Many MDS cases are detected before causing symptoms, simply because a routine blood count showed something unexpected.

Risk classification in myelodysplastic syndromes
MDS isn't managed the same way in every case — management depends on the risk category, determined by the percentage of immature cells (blasts) in the bone marrow, among other factors. It's distinct from acute myeloid leukemia, though some high-risk MDS cases can progress to it over time.
Low-Risk MDS
Management usually focuses on treating symptoms — such as anemia — with close monitoring, without the need for immediate intensive treatment.
Intermediate-Risk MDS
Requires more frequent evaluation and, in some cases, treatment aimed at controlling the disease before it progresses.
High-Risk MDS
Has a higher chance of progressing to acute myeloid leukemia, so treatment tends to be more active, and in selected cases, bone marrow transplant is considered.
Relationship to Acute Myeloid Leukemia
MDS and AML share an origin in the bone marrow, but are distinguished by the proportion of immature cells present. A precise evaluation determines which diagnosis applies to your case.
