Signs that warrant an evaluation:
Bruises that appear easily
When bruises appear with no injury to explain them, or are larger than expected, low platelets are usually the cause.
Petechiae — red or purple spots on the skin
These small spots, usually on the legs or arms, appear when there are too few platelets to seal off the smallest blood vessels.
Gum or nose bleeding with no clear cause
Bleeding that didn't used to happen, now occurring frequently, warrants a platelet count evaluation.
An incidental finding on a routine blood count
Many thrombocytopenia cases are detected before causing visible symptoms, simply because a lab test showed a low count.

Causes of thrombocytopenia I've treated
Low platelets can have an immune origin, be drug-induced, or relate to another condition — identifying the exact cause determines the treatment and its urgency.
Immune Thrombocytopenic Purpura (ITP)
The most common cause in adults, where the immune system mistakenly destroys platelets. In adults it's usually a chronic condition requiring ongoing management, unlike cases in children, which often resolve on their own.
Drug-Induced Thrombocytopenia
Certain medications can lower the platelet count as a side effect — identifying and, when possible, discontinuing the responsible medication usually resolves it.
Gestational Thrombocytopenia
A mild decrease in platelets that can occur during pregnancy, generally without causing bleeding or requiring treatment, but which warrants follow-up to rule out more serious causes.
Thrombocytopenia Secondary to Another Condition
In some cases, low platelets are a sign of another underlying condition — of the bone marrow, liver, or immune system — so the evaluation looks for whether something else is behind it.
